What is a HMQ (Hazardous Materials Questionnaire) and what is this information used for?
The HMQ is a form the City uses to collect basic information on businesses and facilities within the City. The information requested gives us a better idea about the types of activities, associated chemicals and potential hazards that are located in the areas nearest to our drinking water wells (Wellhead Protection Zones 1, 2, and 3). The information collection is required by our Wellhead Protection Ordinance. View more information about the program or the Wellhead Protection Zone.
