One reason for this can be that Acrobat
Reader opened the document into a separate Reader
window. This will not allow the online submit button to work. Below tells how to
set that preference.
Go to "Edit--preferences" then select the Internet category on the left
side list. On
the right, select the check box option to "Open PDF in browser" as it is
specified in your version.
If this is already checked and not working as expected, there could be many
variables with your system we can not predict, but there is another remedy that
has been successful. Uninstalling and reinstalling Reader, then setting the
functionality as described has worked.

It is very likely that your version of Acrobat Reader is less than 4.0.
You can check this by opening Acrobat Reader, clicking on "Help" and selecting
"About Acrobat Reader" from the menu. If the version number is less than
4.0, you
may download a free copy of the latest version of
Acrobat Reader.
If you have version 4.0 or greater, it is possible Acrobat Reader is not working
properly on your machine. Once again, download the latest version of
Acrobat Reader.
There are many reasons why this is slow. The most common reason is your
connection speed to the Internet. If you are using a dial-up modem to
access the Internet, it may take 10 - 15 minutes for the online application to
download to your computer.
